Cremation Forms Changes from September 2024
From September 2024, Form Cremation 4 is no longer required for cremation certification in England and Wales. The confirmatory medical certificate (Form Cremation 5) was already removed, and now Form Cremation 4 has also been made obsolete as part of changes to simplify the process.
Key Changes
- No Medical Certification Required: Doctors are no longer required to complete Form Cremation 4. Instead, the cremation process now relies on the Medical Certificate of Cause of Death (MCCD), which is issued for all deaths and used for death registration.
- Medical Referees’ Role: Crematoria medical referees still have a duty to ensure that the cause of death is clear and there are no concerns requiring coroner referral.
- Coroner Referrals: The existing rules on reporting deaths to the coroner remain unchanged. Any unnatural, violent, or unexplained deaths must still be referred.
What This Means for Medical Practitioners
- No additional cremation paperwork is required beyond the MCCD unless the coroner is involved.
- Timely completion of the MCCD remains crucial to avoid unnecessary delays for bereaved families.
- Best practice remains to provide a clear cause of death, avoiding vague terms like “old age” unless it meets accepted criteria.
