When patients request GPs to complete forms, it’s important to distinguish between those that are mandatory and those that are not. Below is a list of mandatory forms that GPs are generally required to complete, as well as non-mandatory forms where GPs are not obligated to provide support.
These forms must be completed as part of official processes, usually by government agencies or organisations with statutory authority. Failure to complete these forms may impact the patient’s ability to access benefits or services.
DWP – PIP (Personal Independence Payment) Forms
GPs are required to provide the necessary medical information to support a patient’s claim for Personal Independence Payment.
DVLA – Medical Examination
If a patient is applying for or renewing a driving license and the DVLA requires a medical examination, the GP must perform the exam as requested.
DVLA – Medical Report
If the DVLA requests a medical report on a patient’s fitness to drive, the GP must complete and return the report.
DWP – Universal Credit
If the Department for Work and Pensions (DWP) requires information for a Universal Credit claim, GPs must provide the relevant medical information.
DWP – Health Assessment
If the DWP requests a health assessment for the purposes of benefit entitlement or continuation, GPs are required to provide relevant health information.
GMC Guidance on Cooperation with Healthcare Providers
GPs must cooperate with other healthcare providers, as per the General Medical Council (GMC) guidance, which may involve providing patient information as part of a multidisciplinary approach to care.
NHS Blood and Transplant Request for Patient Information
GPs are required to cooperate with NHS Blood and Transplant if they request patient information in relation to organ donation or transplantation.
Council Tax Exemption Form
If a patient claims exemption from council tax due to a medical condition, GPs are required to complete the relevant form.
Criminal Injuries Compensation Form
GPs are required to provide medical evidence when a patient is applying for compensation for injuries sustained in a criminal incident. This includes completing forms to support the claim.
These forms are not legally required to be completed by GPs. While GPs may choose to assist patients, they are under no obligation to do so.
Student Finance Disability Form
GPs are not required to complete disability forms for student finance purposes unless they specifically choose to provide supporting evidence.
MOD Veterans UK Forms
GPs are not mandated to complete forms related to veterans’ healthcare or benefits, but may do so if they choose.
Ofsted Reports
GPs are not required to complete or contribute to reports for Ofsted or other educational bodies.
Firearms Medical Information
GPs are not obligated to provide medical information for patients applying for firearm licenses.
Fostering/Adoption Medicals
GPs are not mandated to complete medicals for fostering or adoption purposes, although they may do so if requested.
Travel Insurance and Holiday Cancellation Forms
GPs are not required to fill out forms related to travel insurance or holiday cancellations unless they choose to provide the information.
Health Insurance Claim Forms
GPs are not required to complete or submit claim forms for private health insurance providers.
Pension Forms – Health Forms from Patients
GPs are not required to complete health forms for pension purposes unless they opt to provide supporting information.
HGV Medicals
GPs are not legally required to conduct or complete medicals for Heavy Goods Vehicle (HGV) licenses unless they have a contractual obligation to do so.
Forms to Participate in Sporting Events
GPs are not required to complete health forms for patients participating in sporting events unless they wish to support the patient.
Disabled Persons Bus Pass Form
GPs are not obligated to complete forms for disabled persons’ bus pass applications.
Camp America Health Forms for Patients
GPs are not required to complete health forms for patients applying to participate in Camp America or similar programs.
SY Fire & Rescue Health Forms for Fire Service Applicants
GPs are not required to complete health forms for patients applying to join the fire service unless they choose to do so.
Army Medical Reports
GPs are not legally required to complete medical reports for patients applying to join the army, though they may provide a report if requested.
Seat Belt Exemption Form
GPs are not required to complete medical forms for patients seeking a seat belt exemption.
British Skydiving Forms
GPs are not required to provide medical reports for British Skydiving or similar activities unless they opt to do so.
ECO4 Flex Form – Council
GPs are not required to complete forms related to financial assistance for heating (ECO4 flex) through the local council.
