Strengthening PCNs

The goal is to continue building Primary Care Networks that help GP practices work together efficiently and coordinate with other health services like hospitals and community care.

Focus on Patient Care

PCNs will focus on improving the quality of care by increasing the use of technology and sharing resources between practices.

New GP Roles and Support Staff

The contract supports the introduction of new roles such as General Practice Assistants (GPAs) and Digital Leads, aimed at easing GPs’ workloads and improving patient care.

Participation Requirements

GP practices are automatically included in the 2024/25 Network Contract DES if they participated in previous years unless they choose to opt-out.  New or existing practices can join or form a PCN by following a formal process that includes submitting key information about their patient population and services provided.

Organisational Requirements:
PCNs must have a clear structure, including a Clinical Director and a Nominated Payee for financial management.

The contract emphasises efficient management, shared records, and clear agreements among member practices.

Service Delivery:
PCNs are responsible for providing services during Network Standard Hours: 6:30 pm to 8:00 pm on weekdays and 9:00 am to 5:00 pm on Saturdays.

Structured Medication Reviews are mandatory to ensure that patients, especially those with complex needs, receive proper medication management.

New Roles:

General Practice Assistants (GPAs): Support GPs by managing administrative tasks and conducting basic health checks.
Digital and Transformation Leads: Improve the use of technology to enhance patient access to care and streamline operations.
Enhanced Practice Nurses: Provide specialised nursing services in areas like diabetes care, wound management, and chronic disease management.

Additional Roles Reimbursement Scheme (ARRS):
This scheme funds additional healthcare professionals like clinical pharmacists, physiotherapists, and paramedics within PCNs to broaden the range of services they provide.

Financial Entitlements:
PCNs receive funding for various aspects of service delivery, including enhanced access services, support for care homes, and general PCN operations. New funding streams include the Capacity and Access Support Payment, which helps PCNs manage patient demand.

PCN Management:
PCNs are expected to regularly review their performance and adjust their operations to ensure they meet the contract’s goals.  Financial entitlements are linked to achieving specific performance indicators and delivering services effectively.

Integration with Broader Health Systems:
PCNs play an essential role in Integrated Neighbourhood Teams (INTs), which bring together different healthcare providers to manage the health and well-being of local populations (30,000-50,000 people). This integration ensures better coordination of care across different sectors.
